PM Sleep Center Share File for Referring Physicians

We are providing a cloud based system to provide you with a fast and a convenient way to receive sleep study result. If you would like to set up an account please contact our Manager via email, Bryank@pmsleepcenters.com, or phone 623-815-8900.

Please include:

  • Provider(s) names
  • Name of contact person who will be responsible for access the portal (I.e., MA, referral coordinator)

– Phone

– Fax

– Email

If you would like one of your patients to have a Sleep Study:

Please complete our Sleep Center Order Form and fax it to us at: (623) 815-8901.

For your convenience PM Sleep Center will obtain any Prior Authorizations necessary and will contact your office if any additional information is needed.

Please include:

    • Patient history/physical notes pertaining to the order
    • Patient Demographics
    • Copy of the patient’s insurance card(s)
    • For Titration studies, please send copy of original diagnostic study
